What is VNS therapy?
Vagus nerve stimulation (VNS) uses a small implanted device, similar to a pacemaker, that sends gentle electrical pulses to your brain through the vagus nerve. Once programmed to your needs, the device works 24/7 in the background — delivering antidepressant therapy around the clock.
The vagus nerve connects your brain to many parts of your body and plays a key role in mood regulation. When depression disrupts this communication, VNS helps restore balance through consistent, mild stimulation that most patients cannot feel.
Unlike medications that cause systemic side effects, VNS has:
- No metabolic effects
- No weight gain
- No sexual side effects
- No drug interactions
Who is VNS for?
VNS is an advanced option for individuals with chronic depression (> 2 years) who have tried and failed different therapeutic interventions, such as:
- Psychotherapy
- Multiple classes of antidepressants, such as SSRIs and SNRIs
- TMS therapy
- Esketamine (Spravato)
- Electroconvulsive therapy (ECT)

Authorization & surgery
We work with specialized surgical teams and navigate insurance authorization (typically two months).
The device is implanted in a brief outpatient procedure.

A few weeks after surgery, you return to our office for programming.
Using a special wand, we adjust the device settings over several visits to find your optimal stimulation levels — no additional procedures required.

Ongoing care
Once optimized, most patients need only periodic check-ups.
The device continues working while you live your life.
How VNS differs from TMS
While TMS uses external magnetic pulses during daily office sessions, VNS provides internal electrical stimulation around the clock. Some patients who don’t respond to TMS find success with VNS. However, if you haven’t tried TMS yet, we often recommend exploring that non-invasive option first.
Important: VNS doesn’t prevent you from receiving TMS, MRI scans (with precautions), or ECT in the future. The device can be turned off at any time if needed.

Is VNS safe?
Yes. VNS has been FDA-approved since 1997 for epilepsy and since 2005 for depression. The surgical complication rate is low (infection rate: 1.6%), and the procedure is typically performed as an outpatient surgery.
What are the side effects?
The most common side effects are mild and occur only during stimulation: hoarseness or voice changes, slight cough, or throat tingling. These typically become less noticeable over time. Unlike antidepressants, there are no weight, sexual, or metabolic side effects.
Will I still need medications?
Yes, VNS is an add-on treatment. Most patients continue their antidepressants, though some can reduce doses over time. VNS enhances but doesn’t replace your current treatment regimen.
Can others see the device?
The wire in your neck isn’t visible. The chest device — about 2 inches wide — may be slightly visible if you have a small frame, similar to a cardiac pacemaker. Surgeons typically place incisions in natural skin folds to minimize scarring.
Can I go through airport security?
Yes, metal detectors won’t affect the device or be affected by it. Move through at a steady pace and stay at least 16 inches from security equipment. You’ll carry a device identification card.
What if I have a pacemaker?
VNS may affect other implanted devices. Discuss any existing implants with your surgeon during consultation.
Can I control the device?
You’ll receive a personal magnet that can immediately stop stimulation if needed. Your doctor can adjust settings using a programming wand during office visits.
How long before I see results?
VNS is a long-term treatment. While some notice changes within months, significant improvement typically occurs at three to nine months, with maximum benefit around 18 months. The effects are durable and long-lasting.
